245D Waivered Services:
o 24-Hour Emergency Assistance - 24-hour emergency assistance provides clients an immediate response, problem solving, and on-call counseling for assistance at a client's home due to a health or personal emergency.
o Adult Companion Services - Adult companion services provide supervision, non-medical care, and socialization to an individual that is 18 years old or older.
o Homemaker services - Eligible homemaker services range from light household cleaning to household cleaning with incidental assistance and assistance with activities of daily living.
o Individual Community Living Support - Individual community living support (ICLS) provides assistance to older adults who need reminders, intermittent or moderate supervision, and physical assistance to remain in their homes.
This service is provided to clients on the:
- Elderly Waiver
- Community Alternative Care (CAC) Waiver
- ICLS covers support and assistance for adults age 65 and older in the following categories:
- Community engagement
- Health, safety, and wellness
- Household management
- Activities of daily living
- Adaptive support services
- Active cognitive support
o Individualized Home Support - Individualized home support services provide adults or children who live in their own homes assistance and supervision. A staff member provides intermittent or moderate supervision, cueing, direction, instruction, help with daily living activities, and physical assistance. This service is provided to clients on the:
- Brain Injury (BI) Waiver
- Community Access for Disability Inclusion (CADI) Waiver
- Community Alternative Care (CAC) Waiver
- Developmental Disability (DD) Waiver
o Night Supervision - Night supervision services include a staff member providing overnight assistance and monitoring in the client's home. Tasks include reinforcement of independent living skills, safety monitoring, and assistance with activities daily living. This service is available to clients on the:
- Developmental Disability (DD) Waiver
- Community Access for Disability Inclusion (CADI) Waiver
o Personal Support - Personal support services are provided in a client's home or the community in support of achieving potential goals. This helps increase the resident's independence and meet community inclusion goals.
o Respite Care - Respite care is short term care provided in the absence or need for relief of the primary caregiver; who normally provides the care. Care can be provided in the resident's home or the respite provider's home.
